Making an Appointment
Fertility Tasmania welcomes all new and returning patients. To assist you with making an appointment, we have provided the following information for you.
1. Referral
Whilst you do not have to have a referral for an appointment with our doctors, a referral from your GP or other Specialist will attract a Medicare rebate.
Your GP will most likely to forward the referral directly, if not, please bring it with you at the time of the appointment. You can also scan and email it to us.

2. Make an appointment
Please call to make an appointment.
Our opening hours are Monday to Friday 8am – 4pm.
Outside these hours, by special appointment only.

3. Patient registration
Before your first appointment please download and complete the patient registration form (PDF).
Please download, read and sign the patient confidentiality & privacy policy as well as the relevant financial information document gynaecology/fertility or obstetric.
You can scan and email back your completed forms
If this is not convenient you can complete them on the day of your appointment.
4. Visit us
On the day of your appointment, please bring your completed patient registration form if not already returned, or arrive 5 minutes before your scheduled time so that you can fill it out.

There is parking at either Argyle Street parking or Market Place car park.
5. Payment
Payment for an initial and or review consultation must be finalised on the day of the appointment.
We accept all major credit cards and EFTPOS payments.
We can issue immediate refunds through Medicare Online.
If you are in a treatment cycle, payment is requested on the day of your procedure.
